Before You Use This Website

Information on this website is provided for general educational, informational, and administrative purposes. It is not a substitute for professional medical or psychiatric evaluation, diagnosis, treatment, psychotherapy, medication management, or emergency services.
Do not rely on website content to make or delay healthcare decisions. Always seek advice from a qualified healthcare professional who can evaluate your individual circumstances.
Provider-Patient Relationship
Website use does not create a treatment relationship.
A provider-patient relationship is established only after Welch Psychiatric Group, LLC has accepted you for care, required intake and consent documents have been completed, and an appropriately licensed clinician has begun providing professional services.
Viewing the Website
Reading pages, articles, service descriptions, or resources does not create a professional relationship.
Submitting a Form
Sending a contact form, inquiry, interest form, or professional opportunity request does not establish care.
Scheduling
Requesting or reserving an appointment does not guarantee acceptance, diagnosis, prescribing, or ongoing treatment.
Communication
Email, text, voicemail, or portal communication alone does not create a treatment relationship unless care has otherwise been established.

Medical and Psychiatric Disclaimer

Information may be incomplete, general, outdated, or inapplicable to your individual circumstances. Never disregard or delay professional care because of something you read on this website.
No statement on the website guarantees that a particular diagnosis, medication, test, treatment, accommodation, letter, or service will be appropriate or provided.
Emergency and Crisis Use Prohibited
This website, public forms, email, text messaging, voicemail, scheduling tools, and routine portal messages are not monitored continuously and must not be used for emergencies or urgent safety concerns.
Do not use these channels for suicidal thoughts, homicidal thoughts, threats, overdose, severe medication reactions, rapidly worsening symptoms, psychosis, withdrawal, abuse emergencies, or other urgent medical or psychiatric concerns.

Appointments, Requests, and Availability
Appointment availability shown through a scheduling platform may change and is not guaranteed until scheduling is confirmed.
Welch Psychiatric Group may decline, cancel, reschedule, or redirect an appointment when clinically, legally, operationally, or professionally appropriate.
Submission of an appointment request does not guarantee acceptance as a patient, ongoing treatment, medication prescribing, controlled substance prescribing, completion of forms or letters, or any particular outcome.
Separate financial, cancellation, telehealth, consent, and treatment policies apply to scheduled services.
Telehealth Availability and Location
Telehealth services may be provided only when the patient is physically located in a jurisdiction where the treating clinician is legally authorized to practice and when telehealth is clinically appropriate.
Website statements regarding service areas are informational and do not guarantee that care may legally or clinically be provided in every location or circumstance.
Telehealth care is governed by a separate Telehealth Informed Consent and related practice policies.
Review Telehealth Informed Consent →Fees, Insurance, and Payment Information
Fees, insurance participation, reimbursement information, and scheduling availability may change. Information displayed on the website is provided for convenience and does not guarantee coverage, reimbursement, network status, benefits, authorization, or payment by an insurance plan.
Patients remain responsible for understanding their financial obligations and for payment as described in the applicable Financial and Cancellation Policy.
Review Financial and Cancellation Policy →Forms, Letters, and Administrative Documentation
Website descriptions of forms, letters, accommodations, workplace documents, school documents, disability paperwork, emotional support animal documentation, or other administrative services do not guarantee that any document will be completed.
Documentation may require an established clinical relationship, sufficient evaluation, supporting records, clinical justification, additional appointment time, and payment of an applicable fee.
Welch Psychiatric Group may decline requests that are not clinically appropriate, legally supportable, within scope, or supported by available information.
Medication and Prescribing Information
Nothing on this website guarantees the prescription, continuation, dosage, taper, replacement, early refill, or approval of any medication.
Prescribing decisions are made only after appropriate evaluation and are subject to clinical judgment, patient safety, professional standards, pharmacy requirements, federal and state law, and required monitoring.
Controlled-substance prescribing may require additional agreements, prescription-monitoring review, laboratory or drug testing, medical records, vital signs, in-person evaluation, or referral.
Accuracy, Completeness, and Updates
Welch Psychiatric Group strives to provide useful and accurate website information but does not warrant that all content is complete, current, error-free, or suitable for every purpose.
Clinical standards, laws, regulations, insurance participation, service availability, pricing, and practice operations may change without immediate website revision.
We may correct, update, remove, suspend, or replace website content at any time without notice.
Educational Resources and External Information
Educational materials, articles, checklists, resource lists, and links are offered for general information. Inclusion does not constitute endorsement, guarantee, referral, or representation that a resource is appropriate for a particular person.
Users are responsible for independently evaluating external resources and seeking individualized professional advice.

This website may link to or integrate with third-party scheduling, telehealth, patient portal, payment, mapping, analytics, social media, educational, and communication services.

Electronic Communications
By providing contact information, you authorize Welch Psychiatric Group to respond through reasonable administrative communication methods, subject to your preferences and applicable law.
Email, SMS, voicemail, and ordinary website forms may not be fully secure. Do not include highly sensitive clinical information unless specifically instructed to use a secure method.
Message delivery, receipt, and response times are not guaranteed. Clinical communication should occur through the secure patient portal when directed.
Artificial Intelligence, Automated Tools, and Chat Features
If the website later includes an automated assistant, chatbot, symptom screener, artificial intelligence feature, or similar tool, its output will be for general informational or administrative purposes only unless expressly stated otherwise.
Automated output may be incomplete or inaccurate and does not constitute medical advice, diagnosis, treatment, prescribing, crisis support, or communication with a licensed clinician.
Do not submit emergency or highly sensitive information to an automated tool unless the feature expressly identifies itself as a secure clinical service.
Privacy and Protected Health Information
General website data is addressed in the Website Privacy Policy. Protected health information related to patient care is addressed in the HIPAA Notice of Privacy Practices.
These Terms do not replace either privacy document.
Intellectual Property and Permitted Use
The website and its text, design, layout, branding, logos, photographs, graphics, illustrations, videos, downloads, service descriptions, and original materials are owned by or licensed to Welch Psychiatric Group and are protected by intellectual property laws.

Website Availability and Technical Issues
The website and its features are provided on an “as available” basis. Access may be interrupted by maintenance, outages, internet failures, cybersecurity events, platform changes, third-party failures, or circumstances beyond our control.
Welch Psychiatric Group does not guarantee continuous, timely, secure, or error-free access and is not responsible for losses caused by inability to access or use the website.
No Warranties
To the fullest extent permitted by law, the website and its content are provided “as is” and “as available,” without warranties of any kind, express or implied.
Welch Psychiatric Group disclaims implied warranties of merchantability, fitness for a particular purpose, title, noninfringement, accuracy, completeness, security, availability, and freedom from harmful components, to the extent permitted by law.
No Guarantee of Outcomes
Welch Psychiatric Group does not guarantee acceptance as a patient, appointment availability, diagnosis, medication recommendations, prescription approval, treatment response, symptom improvement, insurance reimbursement, accommodation, documentation, or any other result.
Healthcare outcomes vary and depend on numerous factors outside the control of the website and practice.
Limitation of Liability
To the fullest extent permitted by law, Welch Psychiatric Group, LLC and its owners, clinicians, employees, contractors, agents, affiliates, vendors, and licensors will not be liable for indirect, incidental, consequential, special, exemplary, or punitive damages arising from or related to website access, use, inability to use, reliance on content, third-party services, communication delays, technical failures, data loss, or unauthorized access beyond reasonable safeguards.
Nothing in these Terms excludes liability that cannot lawfully be excluded or limited. These website limitations do not alter professional duties arising from an established provider-patient relationship.
Indemnification
To the extent permitted by law, you agree to defend, indemnify, and hold harmless Welch Psychiatric Group, LLC and its owners, clinicians, employees, contractors, agents, affiliates, vendors, and licensors from claims, losses, liabilities, damages, costs, and expenses arising from your unlawful or unauthorized website use, violation of these Terms, infringement of another person’s rights, or misuse of website features.
Suspension or Termination of Website Access
Welch Psychiatric Group may suspend, restrict, or terminate access to all or part of the website at any time when reasonably necessary for security, maintenance, legal compliance, operational needs, or suspected misuse.
Termination of website access does not by itself determine or terminate an established clinical relationship, which is governed by separate clinical, legal, ethical, and practice policies.
Accessibility
Welch Psychiatric Group seeks to provide an accessible website experience. If you encounter an accessibility barrier or need information in another format, contact our office and describe the page, feature, or content involved.
Accessibility requests may be submitted to admin@welchpsychiatricgroup.com or by calling 720-619-6082.
Changes to the Website and These Terms
Welch Psychiatric Group may revise these Terms, website content, features, services, or links at any time. Updated Terms become effective when posted unless another date is stated.
Continued use of the website after updated Terms are posted constitutes acceptance of the revised Terms.
Governing Law and Venue
These Terms and disputes arising from website use are governed by the laws of the State of Colorado, without regard to conflict-of-law principles, except where federal law controls.
To the extent a dispute is not subject to another enforceable agreement and venue may lawfully be specified, exclusive venue will lie in the state or federal courts serving Douglas County, Colorado.
Severability, Waiver, and Assignment
If any provision of these Terms is found unenforceable, it will be modified to the minimum extent necessary or severed, and the remaining provisions will remain in effect.
Failure to enforce a provision is not a waiver of that provision or any other right. You may not assign rights or obligations under these Terms without written consent. Welch Psychiatric Group may assign these Terms in connection with a lawful business transition, reorganization, or transfer.
Entire Agreement for Website Use
These Terms, together with the Website Privacy Policy and any other policies expressly incorporated by reference, constitute the entire agreement regarding general website use.
They do not replace patient-specific informed consents, financial agreements, HIPAA notices, medication agreements, telehealth consents, or clinical policies that apply after care is established.
